It is advised to keep P (provider) charts and patient/employee charts separate and not to set both chart types on a chart, nor ever merge them. Clinicians should have a provider (P) chart as well as their separate regular employee/patient chart. The Provider chart is what is used on encounters to set the performing provider/location on the encounter and store other information about the Provider on a Provider level, but their employee/patient chart is what is used if they have to be seen or treated for medical care for any reason, medical history information, etc.
